Description
Coordinates patient admissions from the point of referral through discharge for the Rehabilitation Unit. Responsible for coordinating patient's stay with the patient's payor source. Maintains logs and assists in analyzing referral sources. Participates in data entry for CMS as well as data collection and entry for F.I.M. follow up. Participates in marketing responsibilities. In conjunction with Social Worker, serves as care coordinator for all patients. Covers for the social worker in his/her absence. Participates in organizational committees. Covers the unit for the Program Manager in his/her absence.
Requirements
Current license in the State of Ohio.
BS preferred.
Minimum of two (2) years Rehab or Case Management experience.
CRRN preferred.
One year's experience in case management for acute rehabilitation programs preferred.
