Minimum Qualifications
Requires a Bachelor's Degree in social work, psychology, counseling, nursing,occupational therapy, vocational rehabilitation, therapeutic recreation, or humanresource management (human service track), AND two years of experience working withspecial population groups in a direct care setting. OR a Master's Degree in one of thefields listed above.

Case management includes engagement in a hopeful, trusting relationship; assistance obtaining basic supports , coordination of services, ongoing assessments to determine strengths and barriers and crisis prevention and intervention. Some travel may be required.
