Evangelical Homes of Michigan (EHM) is a multi-faceted health and human service organization that provides health care and home based services throughout southeast Michigan. Founded in 1879, EHM is one of the largest nonprofits of its kind and one of the oldest in the state, serving more than 3,200 older adults annually.
We are excited and eager to welcome a new part time Social Worker to our Medicare-certified home health care division, Shared Services Home Health Care, servicing the Oakland, Washtenaw, and Macomb Counties.
As the Social Worker, your primary purpose is to assist in planning, organizing, implementing, evaluating, and directing the overall operation of the Social Work in accordance with current federal, state, and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by the Sr. Executive Director of Home & Community Based Services, to assure that needs of the clients are met/maintained on an individual basis.
Essential Job Duties for the Social Work include, but are not limited to:
Meet with clients and families to determine appropriate pathway for the home care client.
Accurately complete and assess the MDS for home care clients, as well as requiredEstimated Salary: $20 to $28 per hour based on qualifications.
