Summary:
Under the guidance of the CFO, the Director of Care Management offers support to the Care Management staff through clinical mentoring, program development and process improvement, as well as, coordinates the care and services of patients with identified need for intervention across the continuum of cares. Develops, facilitates and executes a comprehensive plan of care based on ongoing assessments. The Director acts as liaison between patient, family, healthcare team and outside community agencies. The Director also provides oversight of the clinical staff in the Care Management department.
EDUCATIONAL/LICENSING:
Current licensure as a Registered Nurse in the State of Oregon. Current BLS.
QUALIFICATIONS/SKILLS:
3-5 years of experience as a nurse in an acute care setting. Experience in process improvement and program development. Demonstrated competency in teaching skills. Supervisory experience preferred. The Director of Care Management plans and implements clinical education based on identified learning needs and emerging healthcare trends. Interprets standards of practice, benchmarks of care as related to care management in the acute care setting. Experience as a clinical preceptor. Manages specific populations, as well as, case management of individual patients. Effectively facilitates multidisciplinary teams and small groups.
