Description
SUMMARY
Responsible for facilitating and coordinating the PPS (Prospective Payment System) in collaboration with the interdisciplinary team. This position involves collecting, coordinating and evaluating clinical information, coding this information and entering it into the computer system. This position also monitors patient care plans to ensure compliance with applicable federal/state guidelines, and assists with the development and implementation of quality assessment and improvement process.
REQUIRED QUALIFICATIONS
License to practice professional nursing in the State of Michigan
Demonstrated knowledge of computer systems; experience with data entry, processing and retrieval
Demonstrated ability to communicate and express ideas logically and persuasively, orally and in writing
Knowledge of local Home Care Agencies and the services provided by these agencies
Knowledge of Medicare and Medicaid guidelines and regulations
Knowledge of coding systems
Ability to assess patient charts and transfer information to assessment documents
PREFERRED QUALIFICATIONS
At least 3 years experience in Case Management with a clinical/medical background
