The Bereavement Coordinator is a hospice-trained interdisciplinary team member who ensures grief counseling for the bereaved. He/she will oversee and ensure compliance of the bereavement department with corporate, state, and federal mandates.
1. Supports agency and corporate policies, goals and objectives.
Promotes a positive working relationship between agency and corporate personnel.
Promotes hospice philosophy.
Attends and participates in staff meetings.
Communicates identified needs and potential solutions to supervisor.
2. Ensures regulatory compliance in record keeping and all other bereavement activities.
Ensures assessment of bereavement needs according to corporate, state, and federal regulations.
Develops, implements, and documents a plan of care according to identified needs.
Maintains bereavement records, as required by guidelines.
3. Trains staff, volunteers, and the public sector through in-services and public speaking.
Schedules presentations to community groups.
Recruits and retains volunteers and staff to provide grief support.
Ensures that volunteers and staff receive appropriate orientation and training, consistent with acceptable standards of hospice practice.
Oversees bereavement follow-up by staff and volunteers.
4. Functions as an IDT member.
Attends IDT meetings and communicates needs of the pre-bereaved and current bereaved to IDT members.
Works closely with Social Worker following the pre-bereavement assessment to ascertain needs.
Prepares service reports, as requested by management.
5. Ensures provision of grief support to the bereaved.
Provides, or arranges for, grief counseling visits to meet the needs of the pre-bereavement plan of care and post death bereavement plan of care.
Designs/obtains materials for distribution to families eligible for hospice bereavement services.
Organizes and conducts a minimum of two (2) grief recovery support group sessions per year.
Assists with organizing and conducting annual memorial services for the entire census and any or all LTCF memorial services, as requested.
6. Other duties as performed or assigned.
Bachelor of Science Degree in Social Services, Human Services, or related field; or equivalent related experience.
Clinical experience or training related to death, dying, and bereavement.
Strong organizational, interpersonal, communication, teaching, and public speaking skills. Computer experience preferred.
Ability to work with other professionals in related fields.
Computer proficiency in Microsoft Office.
Must be of high integrity including maintenance of confidential information including all patient records.
Ability to work a flexible schedule, to include some evenings and weekends.
Has a valid driver's license and auto liability insurance.
Ability to meet physical job requirements as specified by attachment.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
