Upon graduation and obtaining a degree in Nursing from a NLN or CCNE accredited program and
tennessee
state-approved program, applies the nursing process to plan and manage the care of patients, including patient and family education and continuum of care planning. Effectively communicates pertinent patient/family information to health care. team. Participates in finding solutions for variances from patient goals and ensures implementation of solutions. Furthers professional growth by engaging in 6- 10 hours of learning activities annually.
Key Functions and Expected Performances:
1. PLANNING & MANAGING CARE
1. Practice adheres to specialty and organizational standards.
2. Addresses patient safety needs
3. Applies age specific/cultural considerations to care of patient.
4. Organizes and triages patient care - timely, accurate, and acuity determination.
5. Manages patient flow for maximum efficiency and notifies internal resources of changes

2. CONTINUUM OF CARE PLANNING
1. Identifies discharge/transfer needs and initiates plan for movement to next level of care.

4. Acts to resolve systems breakdowns, knowledge, and behavior issues that impact continuum of care needs.
