FUNCTIONS:
The CCM works as an integral member of the Homeless Prevention and Rapid Re-housing Program, with minimal supervision, to provide services on a variety of diverse cases. The CCM will manage a caseload of 20-25 households per month. This position will work closely with the other homeless services, prevention and housing programs and providers. Case management services will be focused on housing and on promoting long-term housing stabilization. The case manager will participate as a full and regular member of the Housing Services Team (HST), the body that will review case updates, approve increases to program participants and review updates and re-certifications. The CCM will provide case management coverage when the HPRP case manager is not available.
REQUIREMENTS:
o Graduation from an accredited four year college or university with a Bachelor's degree in a human services related field.
o A Master's degree in a related field is required.
o At least two years of professional social work/case management experience with the homeless and/or at risk populations.
o Community organizing experience is a plus.
o Fluency in Spanish and English is required.
Other knowledge, skills and abilities:
o Ability to work in a team environment and independently with minimal supervision.
o Ability and desire to engage at risk or homeless clients and develop trusting relationships.
o Knowledge of the principals and practices of case management.
o Demonstrates knowledge of available community resources.
o Knowledge of current homeless, and housing issues and methods/approaches to address issues.
o Ability to analyze case information and to reach sound decisions on the basis of such information.
o Ability to use HMIS, establish and maintain case records and to facilitate data collection.
o Ability to maintain professional ethics and confidentiality of client information.
o Ability to establish and maintain effective working relationships with diverse individuals.
o Ability to schedule and manage workload sufficiently to meet deadlines.
OTHER:
This position requires driving your personal vehicle for company business. Must be privileged to drive according to the criteria set forth by Agency guidelines.
Valid driver's license in jurisdiction of residence.
Vehicle liability coverage if driving personal vehicle. (on occasion, may be required to transport clients in personal vehicle)
May be required to drive a van.
Acceptable Criminal background check.
Negative Drug Screening.
CPR/First Aid Certification must be acquired and maintained once employed.
PRINCIPLE ACTIVITIES:
Works to identify individuals and families targeted for outreach and assessment for homeless prevention and rapid rehousing assistance.
Receives referrals from the Case Review Team (CRT) and meets with the client within three working days of receiving the case assignment. Works with clients to develop and implement housing services plans, to identify housing options and to meet social, health, emotional and economic needs to maintain housing. With the client, formulates objectives, identifies actions and monitors progress in locating and maintaining housing and achieving other client goals.
Assesses clients' eligibility for benefits, determines appropriateness of households for receipt of prevention/diversion funds and recommends amounts. Conducts periodic reassessments of individual service plans, progress and presenting issues in order to identify new goals and modify existing goals as appropriate, but at least every 90 days. Recertifies eligibility for funds every 3 months.
Meet with the client as often as needed to ensure that the service plan is followed and that the information on the household is current, the rent and utilities are paid and that the budget is being maintained.
Submit requests for locating housing and for inspections to the Housing Locator, when appropriate.
Uses automated technology including the Collaborative Service System of Arlington (CSSA) and hard copy files to maintain, update, and report on case data, goal attainment, and outcomes in a timely manner.
Once a unit has been identified by the Locator, coordinate with the client on completing the steps needed to access the unit, visit the apartment and complete the application forms, etc.
Work with the property manager about issues relating to conditions of tenancy, such as disputes with neighbors.
Ensure that clients who might be eligible expeditiously complete the application for Housing Grants.
Provides/coordinates transportation services as needed.
Prepares reports as directed.
Attends meetings as directed.
Work in cooperation with DHS, Health Department and other government and community agencies toward meeting program and client services plans goals and objectives.
Provides linkage, facilitation and transportation to clients as needed in order to assist with obtaining housing.
Complete the HPRP Housing Assessment in CSSA after case closure at three, six and twelve months after closure.
Prepares proposals, reports, and statistics for submission as needed or required.
