Supportive Living Coordinator

In this position you will coordinate and provide supportive living care for a 23 year old man living in his own home.
You must be able to:
Understand,follow and implement a behaviour management plan outlined by a behavioral psychologist; be proficient at documenting and recording required data per the program; must be a team oriented individual and be able to communicate professionally with all of the staff and other providers as needed in order to maintain a high level of consistency in the program environmment; accompany the client to medical appointments and outings in the community as required; understand medication requirements, behavioral interventions as well as the training materials provided; be physically fit and be able to maintain attention and focus by staying alert at all times as this client require constant supervision.
Self motivation with a positive attitude is a must.
